When can I schedule a medical consultation?
You can schedule your medical consultation after you have completed the medical intake form. We will receive an automatic notification and one our team members will contact you for scheduling.
You will be able to enter the date and time you are available on your medical intake form.

How does the doctor contact me?
Once you have scheduled your medical consultation, Dr. Colantonio will text you first to confirm your availability and then call you from (516) 521-8980. Please allow a 15-minute grace period after your scheduled appointment as
Dr. Colantonio may also have a patient in office.
During my medical consultation, please inform Dr. Colantonio:
- If you are taking or planning to take any medications
- If you have an upcoming procedure or surgery
- If you have medical conditions or family history of any medical conditions
- If you have any drug allergies
- If you have previously taken Semaglutide, Tirzepatide or any other weight loss medication.
- If you are switching between medications
What if I need to cancel or reschedule?
If you need to cancel or reschedule, please contact us as soon as possible.
After I am medically cleared….
Dr. Colantonio will send a prescription to the specialty pharmacy. Your payment will be processed for your selected program and shipping option selected on your medical intake form, minus the consultation fee ($60). You can also feel free to connect with us if you would like to discuss the program or have more questions.
Your medical clearance expires after one (1) year from the date of your initial consultation.
